Cost: Mitey Mite thru Midget for 2011 is 95.00, Flag is $75.00
Payment method mustbe a check made payable to WGAC or cash.
Any return checks will be assessed a fee of $30.00 no exceptions.
There will be additional Fund Raising to offset the cost.
$40.00 OF THE REGISTRATION FEE WILL BE NON-REFUNDABLE

West Genesee Pop Warner 2011 Fundraiser
This year’s fundraiser program will be a raffle instead of selling
candy.
Every participant’s parent or guardian (NY State law prohibits
anyone under the age of 18 to sell raffle tickets) will be required
to sell a minimum of 20 raffle tickets priced at $5 each.
There will be a $50.00 deposit (Check please) for the tickets collected
at the time
of registration with a maximum of $100 deposit per family with multiple
participants.
Tickets will be handed out at the time of registration.
Drawing will be held on Saturday September 10th at the Pep Rally
• 20 numbered tickets will be handed out at registration.
• Only adults (18 and older) are allowed to sell tickets
• Please return your sold ticket stubs and money as soon as you can.
(The field will be open on Wednesdays and Thursdays from 5:30PM to
7:30PM starting June 14th.)
• You are responsible to sell all of the tickets.
• If you need additional tickets contact Dom Mazza 263-0599.

• Sales incentives for individual sellers selling more than 20 tickets.
For selling 30 Tickets (10 extra), will receive a WG Football T-shirt
or Cheer T-shirt
For selling 45 Tickets (25 extra), will receive an embroidered WG
Football or Cheer Hoodie
For selling 60 Tickets (40 extra) - $50 Gift card
Special incentive for the most tickets sold over 60 - $100 Gift Card

The refund request form can be filled out at the
Pottery Rd football fields anytime during practice.
THE FOLLOWING ITEMS MUST BE BROUGHT TO SIGN-UPS:
• Need to bring a current 2” by 2” photo. Photos are only good for
1 year.
• Physical from doctor must be dated after January 1, 2011. Also you
must use the
National Pop Warner Medical Form. If you can't get a physical by August
1st,
we will offer an opportunity to have a physical done at the field
for a $10.00.
POP WARNER RULES: NO ONE WILL BE ALLOWED TO PARTICPATE WITHOUT
A PHYSICAL.
• Copy of Final Report Card (June 2011) must have all pages of report
card.
This is for all participants, Flag thru Midget. If your child is below
70 average you will need to have a Scholastic Report filled out by
the principal of the school. Principals will be in the schools until
about July 6.
