(10:00-1:00)
We do not accept credit cards for registration. We accept cash but
prefer checks.
ALL Checks should be made out to WGAC (West Genesee Athletic Club).
Cost: Mitey Mite thru Midget for 2013 is 95.00, Flag is $75.00
Discounts are available for families with multiple participants.
An additional $50.00 (fund raising deposit) will be required at registration.
(See Below) Payment method must be a check made payable to WGAC or
cash.
Any return checks will be assessed a fee of $30.00 no exceptions.
THERE WILL BE NO REFUNDS AFTER AUGUST 1ST
REFUND REQUEST FORM MUST BE FILLED OUT.
(Forms are available at the Pottery Rd Football fields.)
There will be additional Fund Raising to offset the cost of providing
football and cheerleading.
West Genesee Pop Warner 2013 Fundraiser
This year’s fundraiser program will again be a raffle instead of selling
candy.
Every participant’s parent or guardian (NY State law prohibits anyone
under the age of 18 to sell raffle tickets) will be required to sell
a minimum of 20 raffle tickets priced at $5 each. There will be a
$50.00 deposit
(Check please) for the tickets collected at the time of registration
with a maximum of $100 deposit per family with multiple participants.
Tickets will be handed out at the time of registration.
Drawing will be held at the September Pep Rally • 20 numbered tickets
will be handed out at registration.
Only adults (18 and older) are allowed to sell tickets •
Please return your sold ticket stubs and money as soon as possible.
• You are responsible to sell all of the tickets. •

POP WARNER RULES:
NO ONE WILL BE ALLOWED TO PARTICPATE/PRACTICE WITHOUT ALL REQUIRED
PAPERWORK.
THE FOLLOWING ITEMS MUST BE BROUGHT TO REGISTRATION:
• Need to bring a current 2” by 2” photo. Photos are only good for
1 year. • Physical’s from doctor must be dated after January 1, 2013.
Also you must use the National Pop Warner Medical Form. If you can't
get a physical by August 1st, we will offer an opportunity to have
a physical done at the field for a $10.00. Please let us know ahead
of time if you require a physical for your child. • Copy of Final
Report Card (June 2012) must have all pages of report card. This is
for all participants, Flag (if available) thru Midget. If your child
is below a 70 average you will need to have a Scholastic Report filled
out by the principal of the school. Principals will be in the schools
until about July 6. For more information see Steve Mattice at registration.

Returning and first time players: All required Paperwork will be available
at registration or can be downloaded online. Make sure all forms have
2012’s date on it.
Required forms are found at www.popwarner.com (national Pop Warner
site). Click on forms, then Pop Warner forms, then print and fill
out for registration the following, (these forms will also be available
at registration).
1- Medical/Physical form (2 pages).
2- Pop Warner Code of Conduct form (parent and child must sign this
form)
3- Parental Behavior
4- Participant/Player Contract form
