REGISTRATION

We do not accept credit cards for registration.

Next Registration for the 2010 season will be

Saturday June 19th and Sunday June 20th at Shove Park from 10:00 AM till 1:00 PM

THERE WILL BE NO SIGNUPS WHEN THE SEASON STARTS!!!!!!

Cost: Mitey Mite Hawk thru Midget for 2010 is 95.00, Flag is $75.00
Payment method mustbe a check made payable to WGAC or cash.
Any return checks will be assessed afee of $30.00 no exceptions.
There will be additional Fund Raising to offset the cost.
You may opt out of the fundraising but you must pay $50.00 at registration

$40.00 OF THE REGISTRATION FEE WILL BE NON-REFUNDABLE

THERE WILL BE NO REFUNDS AFTER PAPERWORK CERTIFICATION WHICH
IS MID AUGUST REFUND REQUEST FORM MUST BE FILLED OUT.
THAT WILL BE ON THE WEBSITE.

THE FOLLOWING ITEMS MUST BE BROUGHT TO SIGNUPS.

Returning players:

· Need to bring a current 2” by 2” photo. Photos are only good for 1 year.

· Physical from doctor must be dated after January 1, 2010. Also you must use the
National Pop Warner Medical Form. If you can't get a physical by August 2nd,
we will offer an opportunity to have a physical done at the field for a $10.00.

POP WARNER RULES: NO ONE WILL BE ALLOWED TO PARTICPATE WITHOUT A PHYSICAL.

· Copy of Final Report Card (June 2010) must have all pages of report card.
This is for Jr Pee Wee thru Midget. Need 2 copies. If your child is below
70 averageyou will need to have a Scholastic Report filled out by
the principal of the school. Principals will be in the schools until about July 6.

First time players:

· Must bring a copy of the child’s Birth Certificate

· Need to bring a current 2” by 2” photo. Photos are good for 1 year.

· Physical from doctor must be dated after January 1, 2010.
Also you must use the National Pop Warner Medical Form.
