Player Refund Request

Cost: Mitey Mite Hawk thru Midget $95.00, Flag is $70.00 Payment method must be a
check made payable to WGAC or cash.  Any return checks will be assessed a fee of $30.00
no exceptions.  There will be additional Fund Raising to offset the cost.  You may opt out of the
 fundraising but you must pay $40.00.  Bring a garbage bag of bottles for our first fundraiser.

$40.00 OF THE REGISTRATION FEE WILL BE NON-REFUNDABLE

THERE WILL BE NO REFUNDS AFTER PAPERWORK CERTIFICATION WHICH IS AUGUST 19th! 
REFUND REQUEST FORM MUST BE FILLED OUT. 
THAT WILL BE ON THE WEBSITE.          

THESE ITEMS MUST BE BROUGHT TO SIGNUPS.

Returning players:

·        Need to bring a current 1” by 1” photo.  Photos are good for 2 years.

·      Physical from doctor must be dated after January 1, 2008.  Also you must use the National Pop Warner Medical Form.

·        Copy of Final Report Card (June 2008) must have all pages of report card.  This is for Jr Pee Wee thru Midget.
Need 2 copies.  If your child is below 70 average you will need to have a Scholastic Report filled out by
 the principal of the school.  Principals will be in the schools until July 6. 

First time players:

·        Must bring a copy of the child’s Birth Certificate
